Pre-Admission Testing
The pre-admission testing department specializes in the evaluation and preparation of a patient for anesthesia and surgery. The center at JPS fields about 1,000 to 1,200 calls per month including interviews and other patient issues. About 500 appointments are completed in a month. A thorough assessment can also help to avoid delays and cancellations. The staff interview the patient, assist in identifying the need for a preoperative medical consultation, schedule appropriate laboratory testing, chest x-rays and EKGs. Patients are seen from 8 am to 3:30 pm Monday through Friday. There are no nights, weekends, or holiday shifts.

Anesthesia Assessment
Many safeguards are in place to protect our patients and nowhere is this more important than in the surgical patient. The time immediately before the patient goes to the operating room is a time for the nurse to double check anything relating to our patient's care.
Our patients usually arrive one to two hours before their case is scheduled. Here the staff, which consists of nurses and patient care technicians, interview the patients and family, help patients change into hospital attire, check all records to be sure that all orders, labs, x-rays, and EKGs are ready and results are on the chart and all consents are signed. IV access is obtained and any pre-op medications ordered by the physician are given to the nurse. Anesthesia as well as the circulating nurse interviews the patient. This provides an opportunity for the patient as well as their family to ask questions regarding their surgery as well as any other concerns they have. Recovery of minor cases is often done in this department.

Surgical nursing careers today require a broad nursing base, flexibility, stamina and knowledge of complex equipment and procedures that are advancing constantly. Collaboration with various units and individuals as well as good communication skills are essential for the job. Since JPS is a teaching hospital, teaching and sharing information are an important part of the staff's responsibility. The perioperative teams here have the opportunity to hone their teaching skills while advancing their careers.
Our goal is to provide the safest possible environment for optimal patient outcomes. Safeguards to make this possible include regular in-services on patient safety issues, updating and revising policies to keep up with escalating technology. The nursing process is built into our perioperative record.

PACU
The Post Anesthesia Care Unit (PACU) at JPS in Fort Worth, Texas is a 12-bed unit open 24-hours a day, 7 days a week. Patients receiving various types of anesthesia including general, spinal, epidural, local, nerve blocks and conscious sedation are recovered in this area. An average of 25-35 patients are recovered daily. Inpatients, outpatients, same day surgery and some OB patients are recovered in PACU. In addition, the unit serves as an ICU overflow.
This unit utilizes the latest technology in cardiac monitoring and supplies. This is an excellent facility to acquire nursing skills necessary to treat the most complex medical as well as trauma patients. Staff here are receptive to new employees and accustomed to teaching new people. PACU staff have an average of more than10 years experience and provide quality health care to complex surgical patients on a daily basis.

Endoscopy
The Endoscopy department at John Peter Smith Hospital is designed for perioperative management of patients needing diagnostic and therapeutic gastrointestinal and respiratory endoscopy as well as minor gastrointestinal procedures. The department has state of the art equipment including a flouro unit for ERCPs and bronchoscopes. The endoscopy department manages procedures for adolescents, adults and geriatric inpatients and outpatients on an elective, urgent, and emergent basis. The scope of patient care is to provide perioperative nursing care related to the performance of endoscopic gastrointestinal and respiratory procedures and assist the physician in performing needed procedures.

Internships/Training
Twice a year, JPS offers a six month postgraduate program for nurses desiring to become perioperative nurses. The program is based on AORN's Perioperative Course 101. Classes begin in February and July. New graduates as well as experienced nurses are invited to apply. The class size is limited to 3 to 4 students. We believe that this gives the students the best opportunity to experience the maximum learning experience, and makes JPS an employer of choice among perioperative nurses. The students work four 10 hour days for the most part. There are shift rotations at the end of the program for one week.
