Employee & Family Health Insurance Benefits

MEDICAL BENEFITS are available to all full-time and part-time employees and their families. The medical plan gives you the choice of utilizing two levels of In-Network Providers to maximize the benefit you can receive or choosing Non-Network Providers which increases your out-of-pocket costs significantly.  In-Network consists of the JPS Provider network in addition to CIGNA’s Open Access Plus Network of providers.  CIGNA is administering the medical plan, providing customer service, and processing your claims.

DENTAL BENEFITS are available to all full-time and part-time employees and their families through either the JPS Dental Provider network or CIGNA Dental’s network.  CIGNA is administering the dental plan, providing customer service, and processing your claims.  For a summary of benefit limits and out-of-pocket expenses, click here.

VISION/EYECARE BENEFITS are available to all full-time and part-time employees and their families.  VSP offers three levels of benefits with the most coverage offered through the Signature Plus which provides coverage for an eye exam, glasses or contacts, or an eye exam and two pairs of glasses, or an eye exam and two sets of contacts for a 12-month period.  For a vision plan comparison and summary of benefit limits and out-of-pocket expenses, click here.

HEALTH CARE FLEXIBLE SPENDING ACCOUNT is available to all full-time and part-time employees. This benefit offers you the opportunity to pay for non-reimbursed health care expenses with pre-tax dollars.

DEPENDENT CARE FLEXIBLE SPENDING ACCOUNT is available to all full-time and part-time employees. This benefit offers you the opportunity to pay for dependent care expenses with pre-tax dollars.

BENEFITS CREDIT is available to full-time and part-time employees. Beginning January 1, 2010, JPS provides $180 annually ($7.50 semi-monthly) to employees towards the purchase of benefits or to be taken as extra cash. The annual amount will be prorated for new employees hired after January 1. 

LONG TERM DISABILITY (LTD) is available to all full-time employees and is paid 100% by JPS. Your benefit is 60% of basic monthly earnings after 3 months of disability, with a maximum monthly benefit up to $10,000. Your coverage is effective the first of the month following the effective date of full time status.

SHORT TERM DISABILITY (STD) is available to full-time employees and is 100% employee paid. If you become disabled for 30 days because of an illness or accident, the policy will pay you 60% of your salary tax-free for the next 9 weeks.  Your coverage is effective the first of the month following the effective date of full time status.

CANCER/CRITICAL ILLNESS INSURANCE is available to full-time and part-time employees and is 100% employee paid. If you or your dependents are diagnosed with cancer, heart attack, stoke, major organ transplant, permanent paralysis, end stage renal failure or by-pass surgery the policy will pay you a lump sum payment of either: $5,000, $15,000 or $25,000. The payment is made in full at the time of diagnosis and you may use the benefit payment for anything you wish.  Enrollment for this benefit is only during annual enrollment time.

Financial Security Benefits 

WORKERS COMPENSATION allows employees who are injured on the job to file a workers compensation claim. If you experience a work-related injury or illness, you must report it immediately to your supervisor and Occupational Health.

RETIREMENT (PENSION) PLAN is an automatic enrollment benefit provided to eligible full-time and part-time employees after one year of continuous eligible service, and attaining 21 years of age, effective with the plan year beginning 10/1/06. Newly eligible employees become automatic participants with the non-elective contribution of 1% of their W-2 earnings plus deferrals (i.e. eligible gross compensation for the most recently completed calendar year).  JPS also contributes to the cost of this benefit at more than 6% of eligible salary expense.  You are fully-vested after 5 years of participation in the plan.  This is a defined benefit plan that is fully-funded based on the actuarial funding method used, with healthy return on investment experience.  Plan year is October 1 to September 30.

TAX DEFERRED ANNUITY (403(b)) is offered to all employees upon hire to help provide retirement income in addition to the JPS Pension. You may defer a portion of your income for retirement and receive current income tax savings. JPS will match 50% of the first 4% of base pay that full time employees contribute. Eligible employees will vest in employer contributions at 20% per year and be 100% vested in employer contributions after five (5) years of service.

457 DEFERRED COMPENSATION PLAN is available to all full time and part time employees.  The plan provides another way to defer Federal Income Tax and set aside additional income for retirement. Employees may participate upon hire.

BASIC GROUP TERM LIFE / ACCIDENTAL DEATH & DISMEMBERMENT benefit pays the cost to insure full-time employees for 1x your annual salary up to a maximum of $1,000,000. The plan also includes Accidental Death, Seat Belt and Accelerated Life benefits, which would pay an additional amount. Your coverage is effective the first of the month following the effective date of full time status.

BASIC GROUP TERM LIFE / ACCIDENTAL DEATH & DISMEMBERMENT Buy-Up allows full-time employees to buy more basic group term life insurance as a post-tax benefit to increase the 1x base salary coverage paid for by JPS to a maximum of 2x base salary.  Coverage is rounded up to the nearest 1,000.  Your coverage is effective the first of the month following the effective date of full time status.

UNIVERSAL VOLUNTARY LIFE INSURANCE is available to full-time (including Weekend Warriors) and part-time employees and is 100% employee paid. This program provides you the opportunity to purchase additional life insurance for yourself, your spouse and your eligible dependents. Benefits include permanent life insurance, cash accumulation, long-term care, living benefits, etc.  Enrollment for this benefit is only during annual enrollment time.

VOLUNTARY GROUP TERM LIFE INSURANCE Full-time employees will be able to obtain up to $500,000 of additional Life Insurance on yourself, $100,000 on your spouse, and $10,000 on each of your children. 100% Employee paid. The insurance is guaranteed issue on employees up to $250,000 and $25,000 on spouses if you enroll when you are first eligible. 

Personal & Family Assistance Benefits 

EMPLOYEE ASSISTANCE PROGRAM is available through CIGNA Healthcare to all employees and their families. The program offers confidential assistance to help with personal or family related issues. The program is also enhanced with work/life family friendly services. CIGNA actually functions as your PCP when you need treatment for these conditions. You should contact CIGNA Healthcare before you receive any services at 1-877-622-4327.  You should not obtain treatment for mental and nervous conditions, chemical dependency or drug abuse until you have contacted CIGNA, except in true emergency situations.  You will be responsible for payment of health services that have not been authorized in advance by CIGNA.

MALPRACTICE INSURANCE is available to employees involved with direct patient care upon employment. There is no umbrella policy at JPS; therefore, it is recommended that you obtain coverage for yourself. As a new employee the hospital will pay your premium up front and deduct from your paycheck in increments as low as $10.00 per pay period until the full amount has been satisfied. After completing one year of employment you may be reimbursed up to 100% of the annual premium.

TUITION ASSISTANCE PROGRAM - JPS Health Network is committed to furthering the education of its employees. You are encouraged to pursue additional education that you believe will improve your work performance. JPS will reimburse you a portion of the costs associated with taking courses at accredited colleges, universities, and technical schools. PERSONAL COMPUTER TRAINING COURSES are available to all employees.

LAWSON EMPLOYEE SELF-SERVICE (ESS) NETWORK SYSTEM is available for self-directed on-line new employee and annual benefits enrollment and access to personal, payroll, benefits, and employment information.  ESS allows employees to update personal information 24 hours, 7 days a week from any Internet enabled computer from anyplace throughout the world. You are provided your Network ID/username and temporary password during new employee orientation.

EDUCATIONAL EMPLOYEES CREDIT UNION membership is available upon employment. E.E.C.U. offers many services including savings accounts, checking accounts, loans, IRA's, etc.  Details on the various checking account options available may be obtained by contacting E.E.C.U. at 817-882-0800 or by going to www.eecu.org.  Payroll deduction is available after you establish your account.

FAMILY MEDICAL LEAVE (FMLA) is offered to full-time, part-time, and pool employees who have at least one year of service and have worked at least 1250 hours in the previous year. FMLA allows time off for you or your spouse, child or parent’s serious health condition or the birth or adoption of a child. Medical certification is required prior to approval of a leave of absence. This benefit is in accordance with the federal Family Medical Leave Act of 1993, which guarantees your job for a maximum of 12 weeks while you are on leave.

PERSONAL LEAVE is available to full-time employees after three months of service.  Leave is taken in 1 week minimum, 6 weeks maximum time periods and is approved, in advance, by the employee’s department director/administrator.

PAID MILITARY ADVANCE is available to full-time (including Weekend Warriors) and part-time employees called into service for annual training or mandatory reserve duty. Pay is up to 80 hours annually for full-time and 40 hours for part-time employees. JPS pays the eligible military leave hours as an advance and deducts the difference between the military leave advance and actual military pay when the employee returns from leave. Military leave is approved, in advance, by the employee's department director/administrator.

Other Benefits 

PAID TIME OFF (PTO) is available for use to full-time and part-time employees after 90 days of employment.  Paid Time Off consists of Vacation and Sick time. Maximum Accrual is two and one-half times your annual accrual rate.  One time cash-out is allowed annually at 90%.

Part-time employees:

  • Up to 40 hours per year (8 hour days)

Full-time Employees:

  • 0 - 5 years of employment, up to 136 hours per year (17 eight hour days)
  • 6 - 14 years of employment, up to 176 hours per year (22 eight hour days)
  • 15+ years of employment, up to 216 hours per year (27 eight hour days)

PAID HOLIDAYS are available to full-time employees upon employment.  JPS observes 6 paid holidays in addition to Paid Time Off, (Memorial Day, Labor Day, Independence Day, Thanksgiving Day, Christmas Day, and New Year's Day). On these days all regular full-time employees will receive 8 hours of holiday pay.

ANNUAL PERSONAL HOLIDAY is a personal observance holiday provided each January 1 to eligible employees for their use, only during that calendar year, for personal, religious, ethnic, national, local, etc. holidays or celebrations of significance to the employee.  The holiday is pro-rated for new employees based on hire date and may not be used in conjunction with vacations or sick time as it is not intended to supplement such benefits. It is not cashed or paid out at employment separation as it is a holiday benefit.  

PAID BEREAVEMENT is available to full-time employees upon employment.  Benefit pays up to 24 hours in the event of the death of a spouse, child, parent, sibling, grandparent, grandchild, father-in-law, and mother-in-law.

PAID JURY DUTY available to all full-time employees upon employment. If selected for jury duty, employee is excused from work without penalty.

SAVINGS BONDS are available upon employment. You may visit www.nbtco.com and click on Savings Bonds to find detailed information about the Savings Bond program.  To enroll or change your contributions to this plan, call National Bond and Trust at 800-426-9314.  They will work directly with our payroll department to set up, change, or cancel your payroll deductions for this plan. 

HEALTH CLUB MEMBERSHIPS discounts are available to all employees (you must present a JPS identification badge) at Womack Gym (817-337-8888), Fitness for Her (817-263-7005) and Larry North (817-336-6784).  Employees may contact these health clubs for discount/location information.

YMCA memberships are available to you upon employment. Monthly fees may be payroll deducted.

DIRECT DEPOSIT is available upon employment for all employees. You may have your paycheck sent directly to your checking/savings accounts by submitting a Direct Deposit Authorization Form to the Payroll Department. It takes up to two pay periods to establish your direct deposit and you will receive a check until that time.

The "T" E-PASS is available to JPS employees with an employer discount to all employees for unlimited transportation on all DART, TRE, trolleys, and T rides. Pass may be purchased in Human Resources and photo/pass issued at the T facility.

Benefit Plan Requirements

JPS Benefit Plans for medical, dental, vision, flexible spending, disability, life insurance, and retirement all have plan documents containing the plan provisions, restrictions, and administrative rules.  In particular, each plan document defines an eligible employee and most often the employee is described by the employee’s status.  The description of status is, in part, based on the minimum number of hours worked per week.  Should a discrepancy exist between this summary of benefits, the applicable governing plan document shall prevail.  JPS Health Network reserves the right to amend benefit plans.

JPS Pension Plan Effective 10/01/06, the JPS Pension Plan states, “A covered employee shall be any employee who receives or, except for an authorized leave of absence, would be receiving remuneration for personal services rendered to the employer, excluding: (a) employees who are not, as determined by the employer, working a minimum of 30 hours per week; (b) employees who are not classified, as determined by the employer, as a resident, part-time (working less than 30 hours per week), temporary or POOL.  Part time employees working 30 or more hours per week will be covered employees.”
