Outpatient Services:​ Caring for you.

JPS Magnolia Health Center is for patients age 60 and older. Our geriatric doctors and nurse practitioners lead a team of nurses, social workers, pharmacists and other team members. We develop a healthy aging plan that is right for you. Senior adults can also receive primary care at any of our community clinics.

Home Visits.​ Bringing healthcare to you.

Home Visits is a special program to support you and your caregivers. Our team can bring complete, quality geriatric care to you where you live. We can come to your home or your assisted living, independent living, or retirement community. If you regularly require services at home, or have a condition that makes it difficult for you to leave your home, you may qualify for home visits.

Professional Consultations.​ Collaborating with providers and caretakers.

Inpatient Geriatric Consultation Services are available regarding patients age 65 and older who are being treated in the emergency department or who have been admitted to the hospital. Services include a complete clinical evaluation of medical, psychological, social and functional status as well as cognitive evaluation and assistance with managing medication. Our geriatric consultation team can help make the move from hospital to home easier for the patient, caregivers and family members and also helps with facility placement when needed.

 

 

 

Long-Term Care.​ Partnering with nursing facilities

The Care Transitions for Long-Term Care Team works to develop partnerships between JPS and skilled nursing facilities in Tarrant County. The team works to help patients make the transition from the hospital to a care setting that meets their needs. The team works collaboratively with staff members at these facilities to provide quality-focused care and interventions.​

 

 

 

Hospital Elder Life Program (HELP). ​ Partnering with volunteers.

The HELP program is an evidence-based patient program that provides an opportunity to tap the skills of volunteers. HELP connects trained volunteers with patients in the hospital who would benefit from a little extra attention during their stay in the hospital. These volunteers enjoy working with older patients and commit to a weekly shift. HELP volunteers create a friendly hospital environment by providing encouragement and companionship to patients as well as provide assistance with activities such as therapeutic activities, walking, exercising, and eating.
