JPS Health Network is proud to announce that two of its transformative community projects, Thrive on Crawford and Casa de Esperanza, have been honored by D Magazine. These awards recognize our innovative approach to addressing social determinants of health, community engagement, and the groundbreaking impact on healthcare delivery in North Texas.
Thrive on Crawford received the Outstanding Medical Real Estate Projects award. This partnership between JPS Health Network and The NRP Group demonstrates innovation by integrating affordable housing with healthcare services, highlighting the commitment to serving the community beyond traditional healthcare services.

The Thrive on Crawford development includes 67 affordable housing units for individuals earning between 30% to 60% of the Area Median Income (AMI) and 14 market-rate units, the opportunity for economic stability for JPS team members, patients, and community members, and tailored resident services, that include educational programs, financial literacy, job readiness preparation, and healthcare screenings.

Casa de Esperanza is a unique collaboration between JPS Health Network and Fort Worth Housing that provides permanent supportive housing for individuals experiencing homelessness. This initiative also offers essential resources to ensure residents achieve stability and security. JPS Health Network received the Achievement in Community Outreach award for its outstanding efforts in connecting healthcare services with the homeless community during this partnership. The effective outreach led to the placement of healthcare workers at the residential complex, assisting residents in accessing primary care physicians, managing prescriptions, and obtaining behavioral health support.
Casa de Esperanza offers permanent housing to 119 individuals who were once experiencing homelessness. It has reduced emergency room visits by 50%, redirecting its purpose of caring for emergent issues among community members. It also equips individuals housed through Casa de Esperanza with education and information to help shape them into productive members of society.
