It is essential to patients that care extends far beyond the walls of a hospital. For those dealing with a chronic or long-standing condition, having access to resources plays a crucial role in the quality of one’s life.
For JPS Health Network patients like Margaret Sinclair, the post-acute care program made a world of difference in managing pain. Sinclair suffered a fall early into adulthood and still deals with the pain years later. As she has gotten older, she has begun developing other health conditions which affect her day-to-day life. Sinclair has been set up for success through the care she’s received at JPS.
Hosts Brian Maschino and Tatiyana Giddings met with Jessy Hovland, Case Manager, Post-Acute Care, and Amy Govea, Program Manager, Post-Acute Care, to discuss the importance of this outpatient service.
What is post-acute care?
Post-acute care is meant to ease the transition between hospital and home. When JPS patients no longer require acute care but aren’t well enough to thrive in the community, they enter the post-acute care program. Here, patients can ensure that they continue to receive support and healthcare services until they are ready for independence.
Why is post-acute care critical?
Post-acute care is essential to our patients because it guarantees access to quality care. Often, when patients are discharged, they experience uncoordinated care, limited access to prescription services, and a lack of understanding of continued healthcare maintenance. Introducing patients to post-acute care significantly increases their quality of life.
What does the JPS post-acute care program do?
The post-acute care program provides exceptional care management for patients by identifying barriers to healthcare. This includes:
- Education
- Giving patients a voice
- Accessibility to in-network providers
- Ensured medication refills
- Skilled nursing facilities, when necessary
How does post-acute care benefit lives?
Post-acute care allows patients to regain control over their lives. Patients learn how to manage their conditions while still having independence. Those who go through post-acute care are less likely to be readmitted to the emergency department, thus improving the overall patient experience.
Allowing patients to go through the post-acute program substantially increases their attitude toward returning home. Post-acute care provides patients with a network of support that makes the transition less overwhelming.
